I'm absolutely not a spuffy fan but it seems unlikely, given the 'games' it was implied they were involved in, that they didn't use some 'blood play'. Buffy's depression would certainly have fed, and been fed by, that fantasy and I doubt Spike wouldn't have wanted to try.
One likes to think he would have been safe, but it was as mutually abusive as I've ever seen onscreen in a fantasy series (sorry RL creeping in there) so it would have come up.

There was certainly no evidence onscreen, but I think it very likely he did, yes.
